A report out Thursday blames poor communication and inefficiency for the spectacular jailbreak by helicopter in Quebec City last June.

Three prisoners who escaped managed to communicate with the helicopter pilot by cellphone -- a device banned in prisons.

The report recommends more thorough searches in Quebec jails.

It also notes that prison officials never fully communicated that they suspected there might be an escape attempt before it happened.

New security measures including posts to prevent helicopter landings in jail courtyards are now in place.

The three prisoners were caught in Montreal two weeks after they fled.
